You are provided here with an orientation to the facility and a review of our evacuation process prior to beginning work with your therapist.
Before the therapist assigned to you begins to work with you, you will be asked to sign the following documents:
Please call us the day before your scheduled appointment to either cancel or rebook. You are able to change your appointment by calling our office and leaving a message at extension '0', which is available 24 hours a day, 7 days a week.
If we do not hear from you the day prior to your scheduled appointment, and you do not attend that appointment, a missed appointment fee may apply. Avoid this fee by calling early!
All records, reports, photographs, audio recordings, or videos acquired during our delivery of services to you are stored in secure facilities, according to the Personal Protection Act (PIPA). Your records are retained for a period of 16 years following the completion of our services to you or 10 years following the age of majority for minors. For information about accessing the contents of your file during this time period, please contact our office and dial extension 0. Requests for your records need to be made in writing to us, there may be limits to what we can release, and there will be a nominal fee associated for the production of the records. Please contact our Privacy Officer, the Clinical Director at x121 for requests for any personal information.
If distance service delivery has been agreed upon by both client and therapist as the best method for appointments, the following must be considered:
If you have any concerns about the service that any of our staff have provided to you, please contact our office at extension 0 and ask to speak to one of our Directors. If, after speaking to a Director, you are not satisfied with the outcome, please contact your funder directly, and the regulatory body with which the therapist is registered:
Please note that some of our team are not governed by a regulatory body. In these cases, we default to the Code of Ethics set out by the BC College of Social Workers.
